Patient Services
Phase 1
Rising Phoenix MCC, will provide a safe, discrete and comfortable environment that enables you to legally obtain your Michigan Registry Card. We will be your advocate for evaluating the qualifying condition necessary and develop and defend your medical file using the safeguards in place with HIPPA. We will help you complete and submit the necessary paperwork required by the state of Michigan.
Certification Requirements
Phase 2
Prior to your scheduled appointment, you will need to complete in full the
"Michigan Medical Marijuana Program" application, also known as MMMP. If you need assistance completing the application, we will be happy to help you. A Physician Certification from the state of Michigan is mandatory, attesting that you will benefit from the use of Medical Marijuana, by qualifying for any one of the approved conditions.

Our Doctors are licensed and board-certified professionals. They are 100% compliant and subscribe to the concept that using Medical Marijuana, can be the best natural pain reliever available. Our doctor's recommendations for the use of Medical Marijuana, have yet to be rejected by the state of Michigan.
